First a small note about the word fetish. It is rather interchangeable in its own way. A fetish to one person may be a normality to one another. I however like to use my imagination, if I watch something I like I would try to find a cheaper alternative and make it myself. For example, the cage is around £600, I can make one for about £70. I suggest that if you want to take part in fetish porn, watch the videos, see how the cages are constructed (you will get a very good view of them) and make one yourself. OK, cages are only a tiny tiny part of it, but it is the best example.
Moving on from equipment is experience. For example, one fetish to me at least is fisting. Some people will enjoy watching this but it takes a large amount of time to actually be able to do it.
